How to Become Eligible for DDS Services?

To become eligible for services you must complete a three-step process:

First, a questionnaire will be completed that will require specific information regarding your goals, vision, and employment objectives etc.

Next, a complete application providing proof of residency, proof of intelligence quotient (IQ) score below 69 before the age of 18 that continues into adulthood, including cognitive and adaptive impairments.

Last, if eligibility is determined, the Service Planning and Coordination Division (SPCD) will pair the person with a Service Coordinator. The Service Coordinator will ensure the person receives appropriate and effective service delivery.
